GESD 2009 Needs Assessment Survey
Dear GESD Employee,
You have been asked to participate in the district's 2009 Needs Assessment Survey. This questionnaire is intended to provide information on how well the organization is performing across a variety of important topics. The effort is being conducted as part of the district’s on-going school improvement process in conjunction with the Arizona Department of Education.
Instructions:
Non-Administrators: GESD employees who are not Administrators have been assigned to complete a specific topic area (Strand). You only need to complete the questionnaire associated with the Strand that was assigned to you. You do not need to complete all five Strand areas. If you are unsure which topic area has been assigned to you, please contact the GESD Research office at
esloat@gesd40.org or 623-237-7185.
GESD Administrators: You are asked to complete all five topic areas (Strands).
Completing the Survey: Before linking to the on-line survey questionnaire, click on the associated 'Open Rubric' link to view/print the rubrics pertaining to the particular topic (Strand) area. You will need to use these rubrics to evaluate your response to each survey question. There is a separate evaluation rubric for each item. Select the response you feel best reflects the district's performance based on the four criteria levels identified in the rubric. To complete the questionnaire, please follow these steps:
1. Open the Rubric file for the Strand you are evaluating.
2. Either print or keep open the Rubric file so you may reference it while completing the questionnaire.
3. Open the questionnaire for the specific Strand you are evaluating.
4. Answer each question based on the statements contained in the associated rubric.
